במסגרת הליך בדיקת הכשירות הרפואית לספורט, אנו אוספים:
- פרטים מזהים: שם מלא, תעודת זהות, שנת לידה, מין
- נתונים גופניים: גובה, משקל, ענף ספורט, מועדון
- אנמנזה רפואית: תשובות לשאלון רפואי מקיף
- ממצאי בדיקה גופנית ומסקנות רפואיות
- חתימה אלקטרונית
- מסמכים רפואיים: אישורי כשירות והפניות לבדיקות
מידע רפואי מוגדר כ"מידע רגיש" לפי החוק ומקבל הגנה מוגברת.

- הפקת אישורי כשירות והפניות לבדיקות נוספות
- תיעוד רפואי ומעקב
- עמידה בדרישות חוקיות ורגולטוריות
- הצוות הרפואי של המכון — רופאים וטכנאים מורשים בלבד
- tor4you — מערכת ניהול התורים, מקבלת פרטי זיהוי בסיסיים ומצב כשירות
- Google Firebase — שירות אחסון ענן לשמירת המידע, בהתאם להסכם עיבוד נתונים
המידע אינו נמכר או מועבר לצדדים שלישיים לצרכי פרסום.
- כל התקשורת מוצפנת באמצעות HTTPS/TLS
- המידע מאוחסן בשרתי Google Cloud באירופה (אזור europe-west3)
- גישה למידע מוגנת באימות דו-שלבי (2FA)
- כל גישה למסמכים רפואיים נרשמת ב-Audit Log
- קישורים למסמכים רפואיים תקפים ל-15 דקות בלבד
רשומות רפואיות נשמרות למשך 7 שנים מתאריך הבדיקה, בהתאם לדרישות החוק הישראלי.
בתום תקופה זו המידע נמחק אוטומטית.
כמטופל, יש לך הזכות:
- לעיין במידע הרשום אודותיך
- לתקן מידע שגוי
- לבקש מחיקה של מידע שאינו נדרש עוד
- לקבל עותק של המידע שלך
לממוש זכויותיך, פנה לממונה על הגנת הפרטיות בכתובת למטה.
המערכת אינה משתמשת בעוגיות מעקב או כלי ניתוח צד שלישי. אין פרסומות.
Mahon 1 Sports Medicine ("the Institute") is committed to protecting the privacy of its patients in accordance with the Israeli Privacy Protection Law, 5741-1981, and its regulations.
This policy explains what data is collected, how it is used, and your rights as a patient.
As part of the sports medical fitness examination process, we collect:
- Identifying details: full name, ID number, birth year, gender
- Physical data: height, weight, sport, club
- Medical history: responses to a comprehensive medical questionnaire
- Physical examination findings and medical conclusions
- Electronic signature
- Medical documents: fitness certificates and referrals
Medical information is classified as "sensitive information" under Israeli law and receives enhanced protection.
- Conducting sports medical fitness examinations
- Issuing fitness certificates and referrals for further tests
- Medical documentation and follow-up
- Compliance with legal and regulatory requirements
- Institute medical staff — authorized physicians and technicians only
- tor4you — appointment management system, receives basic identification details and fitness status
- Google Firebase — cloud storage service for data storage, under a data processing agreement
Your data is never sold or transferred to third parties for advertising purposes.
- All communications are encrypted via HTTPS/TLS
- Data is stored on Google Cloud servers in Europe (europe-west3 region)
- Access to data is protected by two-factor authentication (2FA)
- All access to medical documents is recorded in an Audit Log
- Links to medical documents are valid for 15 minutes only
Medical records are retained for 7 years from the date of examination, in accordance with Israeli law requirements.
After this period, data is automatically deleted.
As a patient, you have the right to:
- Access the information recorded about you
- Correct inaccurate information
- Request deletion of information no longer required
- Receive a copy of your data
